Cloud Care & Coordination delivers Support Coordination to NDIS participants who are either Plan Managed or Self-Managed.

Located in the Hunter Region of New South Wales, we proudly work with participants both locally and across Australia.

What is Support Coordination, and how can we support you?

Support Coordination is a funded service through the NDIS that helps you understand and use your plan in a way that works for you.

At Cloud Care & Coordination, we support you to break down your NDIS plan, figure out what’s possible, and connect with services that align with your goals and lifestyle — whether they’re NDIS providers or local, mainstream supports.

Our aim is to make the NDIS easier to navigate, less stressful, and more empowering.
